Designing a check in a 15 year old will actually do at 6am

Every monitoring system lives or dies on one number, and it is not ACWR. It is the percentage of athletes who complete the morning check in.
Ours is five sliders and a body map. No free text required. No account switching. One screen, no scrolling on a phone, and it closes itself when you are done. The target is under 30 seconds, and we measure whether we hit it.
What we cut
An earlier version asked about hydration, caffeine, menstrual cycle phase and training motivation. All genuinely useful. All of it went, because a longer form got filled in less often and the extra fields were worth less than the missing days.
The optional free text box stayed, because that is where a player tells you their grandmother is in hospital and their sleep score suddenly makes sense.
